Baptism: On this tenth day of October one thousand eight hundred and forty one Bates Cook, son of Sherman P. Barnum, of this Parish, M.D. and Sarah Chamberlin his wife, born on the twenty fourth day of June last past, was baptized, the Parents here subscribing were admitted as the only sponsors by me. Death Record: Bates Cooke Barnum, son of the late Sherman P. Barnum of this Parish, M.D., deceased, died on the twenty eighth day of December, one thousand eight hundred and sixty at the age of nineteen, and was buried on the thirty-first day of the month and year above. Written in the presence of the subscribing witnesses by me. Signed James Reid, Rector. Signatures: B. Chamberlin and O. B. Kemp ( ED NOTE: B. Chamberlin is Bates' maternal grandfather, Brown Chamberlin.)
